Sirianni’s first game as a head coach was on September 12, 2021, against the Atlanta Falcons. He made sure that the Eagles won 32 to 6. Because he was having symptoms, he was put on COVID protocols on December 22. He was taken off of them on December 25.

In his first year as head coach, Sirianni led the team to a 9-8 record and a spot in the wild-card round. The Eagles lost to the Buccaneers 15–31 in the wild-card game. The Buccaneers lost again the next week. Sirianni joined Chip Kelly and Ray Rhodes as the only Eagles head coaches to make the playoffs in their first year of charge.
